Dear Sir/Madam,

The following is an enquiry to your organisation under the Freedom of Information Act legislation;

I would like this information to contact them regarding research I am doing in this field.

1. The name and email contact details of the person in your organisation

responsible for dealing with Freedom of Information enquiries, and

the number of enquiries received in the last year.

2. The name and email contact details of the person in your organisation

responsible for dealing with Data Protection enquiries, and the number

of enquiries received in the last year.

3. The name and email contact details of the person in your organisation

responsible for dealing with Information Governance Toolkit and your

last submission results.

Our Response

I refer to your email of 16 August 2009 requesting information in respect of

NHS East Midlands information governance.

I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000

that we do hold the information that you have requested.

I will address each of your points in turn:-

The name and email contact details of the person in your organisation responsible for dealing with Freedom of Information enquiries, and the number of enquiries received in the last year.

Chris Brady is responsible for dealing with Freedom of Information enquiries.

The email address for FOI queries is FOI@eastmdilands.nhs.uk.

NHS East Midlands has received 96 separate FOI requests to date since January this year.

The name and email contact details of the person in your organisation responsible for dealing with Data Protection enquiries, and the number of enquiries received in the last year.

See above for contact details. NHS East Midlands has so far this year received 5 Subject Access Requests under the Data Protection Act 1998.

The name and email contact details of the person in your organisation responsible for dealing with Information Governance Toolkit and your last submission results.

Fabian Henderson (Fabian.Henderson@eastmidlands.nhs.uk) is responsible for
