Please provide a list of the Serious Untoward Incident reports (for example, the titles of the reports) that have been received at NHS East Midlands on and since 1 January 2009, if possible disclosing further information such as the month and year of the report and the county or unitary authority area of the NHS Trust or GP etc. that generated the report (or the name of the hospital in the case of incidents from hospitals). Where the title of the report does not give these details, please give me a one-line or, for example, a one or couple of sentences description about the nature of the incident that is the subject of the relevant report.

I refer to your email requesting information in respect of Serious Untoward Incidents.
I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) that we do hold the information that you have requested. However, as this information is already in the public domain, we believe that the exemption available to us under Section 21 of the FOIA is engaged in this instance. Section 21 relates to information that is reasonably accessible to the applicant already.
