This is a request under the Freedom of Information Act for any information/documents/material held by you relating to the Serious Untoward Incident at Barlborough NHS Treatment Centre reported in March 2010, and the findings of the subsequent investigation.
I understand that certain material may need to be redacted to preserve patient confidentiality.

Our Response
I refer to your email of 3 November 2010 requesting information in respect of a Serious Untoward Incident at Barlborough NHS Treatment Centre that you say was reported in March 2010.
I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) that we do not hold the information that you have requested.
However, Section 16 of the FOIA places a duty on us to provide advice and assistance where possible. I should therefore advise you that although Barlborough ISTC sits regionally within NHS Derbyshire County within the East Midlands, the lead commissioner for the contract is the Yorkshire and Humber Specialised Commissioning Group http://www.yhscg.nhs.uk/ which is hosted by NHS Barnsley http://www.barnsley.nhs.uk/. Therefore any SUIs should be reported via NHS Barnsley for onward reporting to NHS Yorkshire and the Humber Strategic Health Authority http://www.yorksandhumber.nhs.uk/.
I understand that the patient concerned was a Bassetlaw patient and that Bassetlaw Primary Care Trust (PCT) was made aware of the incident and was involved in the subsequent investigation. However there was no requirement for them to share this information with NHS East Midlands.
