Can you please provide me with a copy of my full medical records (dated approx. Sept 2001 - to date)?
Would you be able to give me an E.T.A on the arrival of my records, please? A Cheque for the amount of £10.00gbp to follow in the post.
Our Response
I acknowledge receipt of your request for information to NHS East Midlands which was received on 21 October 2010.
Under the Freedom of Information Act 2000 (General Rights of Access) that came into force on 1 January 2005, we have a duty to:

Acknowledge receipt of your request within 2 working days;

Respond to your request within 20 working days.
I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) that we do not hold the information that you have requested as NHS East Midlands does not hold patient records. Therefore, we have no requirement of a fee from you.
As a Strategic Health Authority (SHA), NHS East Midlands has four main roles:

We also ensure that local health systems operate efficiently and effectively and that national policy, standards and priorities are met in order to continually improve services for patients and the local communities we serve. As such, we have no direct patient contact.
I would however advise you to contact your local Primary Trust (PCT) or GP surgery for advice as to where your patent records are held.
