Freedom of Information Act request
I wish to make a series of separate requests under the Act. For convenience, I am including them in one email. They are as follows:
Request #1 Please confirm or deny if your Strategic Health Authority (SHA) has instructed its constituent Primary Care Trusts (PCTs) to include indicators relating to pain management in the Commissioning for Quality and Innovation (CQUIN) schemes which they agree with providers.
Request #2 Please supply details of the indicators relating to pain management which your Strategic Health Authority’s constituent Primary Care Trusts (PCTs) include in the Commissioning for Quality and Innovation (CQUIN) schemes which they agree with providers.
Request #3 Please confirm or deny whether your Strategic Health Authority (SHA) has made an assessment of the prevalence of chronic pain in your area. Please supply details of the assessment made.
Request #4 Please confirm or deny whether your Strategic Health Authority has had any communications with (i) its constituent Primary Care Trusts (PCTs) and (ii) the Department of Health on pain management in the last 12 months.
Request #5 Please supply all communications which your Strategic Health Authority has had with (i) its constituent Primary Care Trusts (PCTs) and (ii) the Department of Health in the last 12 months which refer to pain management.
Request #6 Please confirm or deny if you have developed any guidance to support Primary Care Trusts (PCTs) in the commissioning of pain management services. Please provide a copy of the guidance.

I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) that we do not hold all of the information that you have requested.
However, I will address each of your queries in turn:
