Could you please provide me with full Contact details of the Leader of each of the GP Commissioning Consortia in your region now approved by DH as Pathfinder sites (Wave1 + Wave2) by completing a Table for each Consortia as per the attached MS Word Document? If it is easier for you, I’d be just as happy to have the same data provided in MS Excel.
