Please can you provide me with the following information under the FOI:
With regard to non-executive director Kaye Burnett, I would like to request a copy of the entry in her declaration of interests on the application form which the applicant completes to become a NHS Non-executive Director.
Can you provide the dates that Ms Burnett was a member of the Remuneration Committee?
During this tenure, were there discussions about the bonus payments to be made to Executive Directors, eg the 2008/9 bonus payments described in the SHA FOI reply 2010-532?
During this tenure, were the recruitment and retention premia of Executive Directors reviewed?
If they were reviewed, please supply the minutes relating to these reviews.
For the Social Marketing & Communications Framework for NHS East Midlands & GOEM contract dated the 27.04.2010, what was the title or name of the Executive Director whose area of responsibility covered this contract?
The fact that Ms Burnett's company is listed as a bidder for the “Social Marketing & Communications Framework “on the SHA website implies that a bid was received by the SHA, even though no contract was awarded.
Was approval given by the Trust Board or Audit Committee or Chairman for Ms Burnett's company to bid for this SHA contract?
In an SHA FOI reply dated the 6th January 2011, a payment made to Mutual One in 2009 was given as £27,500.
An earlier SHA FOI response in May 2009, 2009-359 on the SHA website, gave a figure of £13,228. Can the SHA confirm which figure is correct please.

The £27,500 figure is correct. Mutual One was paid £13,228.42 in 2008/09 as reported in the earlier FOI request you refer to and a further £14,262.48 in 2009/10, giving a grand total of £27,490.90.
How or where was this contract advertised?

The Mutual One contract was for a review of the NHS Internal Consortium arrangements operating across East Midlands.

The review was undertaken by Mutual One who have experience of operating an internal audit consortia and its evolution to ensure it continues to meet the requisite standards and expectations of its clients.
