I would like to make a request under the freedom of information act. I would like, for each of the GP practices within your PCT’s area:
The name of the practice.
The practice code.
An email address which may be used for submitting freedom of information requests to the practice.

I refer to your email requesting information in respect of GP practices within our PCT areas.
I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) that we do not hold the information that you have requested. However, I understand that Practice names and codes are accessible via the Connecting for Health website at the following link.
