Good afternoon. This request is made under the Freedom of Information Act 2000.
I would like to request details of all incidents reported to you from organisations where you commission services that are categorised as Never Events according to the NPSA framework during the financial year 2010/11 and also for the first quarter of financial 2011/12.
To clarify please provide:
Category of the incident (according to Never Event Framework)
When the incident occurred
Where the incident took place (which hospital)
I would remind you that the never events list expanded from the previous 8 events in 2010/11 to 25 events in 2011/12 and my request is made accordingly.
I have made a similar request to you in the past and you have supplied this information without exemption, as have other SHAs.
