To whom it may concern,
I have been referred onto you by the Department of Health Freedom of Information team who said that you would be able to help with my request. I would like a list of all of the GP consortia/Pathfinders that have already been formed in your SHA area grouped by GP practice membership. What would be useful is the details of the lead in charge of the GP consortia/Pathfinder,their contact details and the address of the registered office if any.
If there are any GP consortia/Pathfinders that are in the process of being formed (i.e not approved) then please provide these details as well.
Our Response
I refer to your email requesting information regarding GP Consortia Pathfinders.
I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) that we do hold the information that you have requested. However, we feel that this information is exempt from provision as it falls within the FOIA exemption available to us under Section 21. Section 21 relates to information that is already reasonably accessible to the applicant and you should be able to find it by accessing our FOIA disclosure log, available via our web site.
However Section 16 of the FOIA places a duty on us to provide help and assistance where possible so I have attached a pdf document containing the information that you require for ease of reference.
